Apply for a Hearing Dog

Applications for a Hearing Dog are currently closed. 

Be sure to check back for open enrollment dates or fill out a notification form below so you don't miss the opportunity to apply. If you are interested in applying, please review our eligibility criteria, program description, and Hearing Dog FAQ's before completing an application.

Fully completed applications are reviewed by the Paragon Application Committee, and decisions are provided to applicants within three months of submission.

Before completing this application, please pause and critically consider the program description and whether the required commitments feel like a good fit for you. If you are at a point in your life to devote the time, energy, and resources required to become one-half of a successful Hearing Dog team, we are excited to begin the journey with you.

Clients who have a Paragon hearing dog nearing retirement or have a Paragon hearing dog that has passed away within the last year are able to reapply for a hearing dog.

Are you a Veteran?
I am a permanent U.S. resident, living full time in the 48 contiguous states (not in Alaska or Hawaii)
I understand that if matched with a hearing dog, I cannot have another dog in my home (except a retiring/retired Paragon hearing dog)
I can legally drive and/or have full vision with corrective lenses
I am able to walk a dog, typically a Lab, on a leash for >30 minutes of brisk walking daily, with a consistent gait and without an assistive device
I can participate at my own expense in all required meetings and trainings onsite at Paragon in Longmont, Colorado
I can participate in virtual meetings and complete assignments in an online Google Classroom
